Produced by the Respiratory Health Network (WA Dept. Health) in consultation with clinicians, key stakeholder groups, consumers and carers, the Asthma Model of Care is an articulation of best practice care for those at risk of or diagnosed with asthma and designed to improve delivery of services for all Western Australians with asthma.

WA Asthma Action Plan Card
A new Asthma Action Plan Card has been released in WA in 2010-2011. The purpose of the Asthma Action Plan is to provide a self management tool. Information for consumers should be reinforced at GP and specialist appointments. Information should be updated at any time there is a change in the treatment/medication regimen. Pharmacists should record all reliever medication provided to asthmatics and provide assessment and referral if necessary.
